1. Complete & Submit the Form
- Fill out the Graduation Application Form with accurate and complete information.

- If you are a student residing outside the region, coordinate with your Adviser or Program Coordinator for assistance
2. Verification & Payment
- Claim your verified form at the Registrar’s Office
- Proceed to the Dean’s Office for approval
